“Just to give you my unsolicited opinion, I hadn't done CPET since fellowship. It has certainly come a long way and the technology now is computerized and much more compact, easily fitting into one of our exam rooms. In fact, we installed the equipment into the exam room that we were using for our Coumadin clinic. The CPET was installed in one afternoon and we were up and running the next day. We do the tests on Tuesdays and Wednesdays and are doing about 12 tests/week. We have had no problem finding appropriate patients amongst the four of us and may need to add another day of testing in the near future. We are also now able to do full PFTs (including diffusing capacity) on all the amiodarone patients we had been sending to the hospital. I don't deny that there is a definite learning curve for the cardiologist. The staff has made themselves extremely available in person and by phone to discuss any of the results. I have found this technology to be very enlightening and have been ordering it on many of my patients who are always dyspneic or have advanced valve disease and we are trying to decide whether or not to refer them for surgery. It has now become a very important part of my everyday workflow, and as I learn more about interpretation and how to integrate the results with other tests, e.g. cath, nuclear, and echo, have found this technology to be very powerful. It clearly has been influential in my clinical decision-making. ”
